In consideration of your acceptance of my entry, I hereby release and forever discharge Atlanta Open-International Taekwondo Championships, MASTER PIUS MARTIAL ARTS, Alpharetta High School, its agents, units, school, competitors, officials, and employees (former or current and in their official and individual capacities) from all claims, claims for attorney's fees, actions, causes of actions, charges of suits or whatever kind or nature asserted against them, by or on behalf of myself or my children, parents, assigns, heirs, representatives, dependents, or otherwise, arising from my use of any property of the Alpharetta High School.
I also release the above from any and all damages which may be sustained and suffered by me in connection with my association with or entry in the above athletic meet or which arise out of traveling to, participating in, and returning from such athletic meet. Furthermore I also release the above from all damages to my property. I, the undersigned, am at least eighteen years of age. I have carefully read the above stated release and covenant not to use. I understand the contents thereof and agree that my execution of the release and covenant not to sue is my own free, voluntary act and deed. This Release is made on the date signed in consideration of the permission granted by MASTER PIUS MARTIAL ARTS to participate in the Atlanta Open Taekwondo Championships. In Witness whereof, I have executed this release and covenant to not sue Atlanta Open-International Taekwondo Championships, MASTER PIUS MARTIAL ARTS, and Alpharetta High School on the day and year signed.
